Central to life at Freedom is access to 24-hour aged care that evolves with you through all stages of your journey. From everyday support for those with minimal care needs (such as assistance with cleaning, nutrition, or social activities), high clinical care, dementia and memory impairment therapy, and palliative care when the time comes. When you make Freedom your home, our team of care professionals and our health and wellbeing programs will care for your individual requirements as they evolve, in the privacy of your Freedom home.
Freedom Aged Care Nursing Leadership
Our dedicated Home Care Community teams are on hand around the clock to care for each and every one of our community members. Freedom Aged Care has a nursing leadership model of management and care provision, meaning each community is led by a Registered Nurse who is your friendly community manager, and leads a dedicated team who become your ‘Freedom family’. All of who work together to service the needs of their community and each individual resident within it.
Your Freedom family will include an expert team including:
- Home Care Community Manager (Registered Nurse/Senior Registered Nurse)
- Clinical Case Manager
- Nursing and Care Team, including Activities/Lifestyle Coordinators, Diversional Therapists and other specialists as required by the community
- Support Service Staff including village Receptionist, Admin team, Maintenance, Cooks and Cleaners
Your health and wellbeing at the center of your community
Your Freedom family will be there to support and care for you, from sunrise to sunrise, even if you require overnight care. Our professional and friendly aged care team are on site throughout the day, and a night care team will be onsite should you require personal assistance or ongoing care. Based on your assessed needs and community care package arrangement, the care team will look after you according to your personal requirements, which may include:
- Showering and hygiene support
- Medication management
- Wound management
- Skin care
- Administration of daily injections
- Catheter care
- Oxygen therapy
Your safety, security and wellbeing is our priority. We pride ourselves in delivering on our promises to care for you and to support your desire to live independently (as much as possible) in your own Freedom home with safety and security.
We take our regulatory obligations seriously in relation to the quality and safety of our care. For residents who receive externally funded care provided through brokerage arrangements, we comply with all relevant aspects of The Aged Care Act 1997. We also work closely with the local medical community of GPs, pharmacies, and allied health providers to maximise your health outcomes. Some of the allied health services we work with are:
